To participate in the Saint Thomas More dance and haunted house, I warrant that my child is in good health and able to fully participate.

I have instructed my child as follows:
1. He/she should behave in an orderly manner throughout the event.
2. It is important that he/she does exactly what the chaperones require.
3. He/she respects the school property and does not purposefully incur any damage.

As parent/guardian, I agree to all the above stated considerations and conditions.

By signing your child up for this event, you agree to indemnify the parish/school of the Archdiocese of St.Paul/Mpls from any claims or lawsuits brought against the parish/school and the Archdiocese of St. Paul/Mpls by myself, my child or others that arise out of any behavior by my child at the event/activity described below. I also agree to pay reasonable attorney fees or expenses incurred by parish/school and the Archdiocese of St. Paul/Mpls in defense of such a claim/lawsuit.

Emergency Medical Treatment: In the event of an emergency, I give permission to transport my child to a hospital for emergency medical treatment and will be advised prior to any further treatment by a doctor or hospital. In the event of an emergency, the number above will be contacted.
